What is botulinum toxin?

Botulinum toxin type A is a purified protein that temporarily blocks the signal between nerve and muscle. Injected into the small muscles of the face, it stops them contracting hard enough to crease the overlying skin.

This mechanism explains exactly what it can and cannot do. Lines caused by muscle movement — frown lines, forehead lines, crow's feet — soften because the muscle stops folding the skin. Lines that are present when your face is completely still are etched into the skin itself, and relaxing the muscle beneath them helps only partially. The sooner you treat a dynamic line, the less likely it is to become a static one.

Should you fly to Turkey for botulinum toxin?

Honestly, no — not on its own. Flights cost more than the treatment, and the effect wears off in three to four months, so you would be flying three times a year. It makes sense as an addition to a trip you are already making, or combined with a surgical procedure where it addresses something the surgery does not. We would rather say that than sell you a package that does not add up.

Am I a good candidate for botulinum toxin?

The best candidates have dynamic lines that appear with expression, healthy skin, and realistic expectations about what relaxing a muscle can achieve.

Botulinum toxin is usually suitable if

  • Your lines appear when you frown, raise your brows or smile, and soften when your face is relaxed.
  • You are starting to see faint lines at rest and want to stop them deepening.
  • You clench or grind your jaw and want the masseter relaxed.
  • You have a gummy smile caused by a strong upper lip elevator.
  • You have excessive underarm sweating that antiperspirants do not control.
  • You are over 18 and in general good health.

Something else will serve you better if

  • Your lines are deep and present when your face is completely still: relaxing the muscle helps only partially, and filler, resurfacing or a lift may be needed.
  • Your concern is loose skin rather than lines: toxin does nothing for laxity.
  • Your brow has already descended: treating the forehead will drop it further.
  • You have a neuromuscular condition such as myasthenia gravis.
  • You are pregnant or breastfeeding.
  • You have an infection or active skin condition at the injection site.

How we verify a facility

How is botulinum toxin injected?

A series of small injections into specific muscles, placed by anatomy rather than by following the lines on the surface.

  1. Assessment of movement: you are asked to frown, raise your brows, smile and squint while your doctor observes which muscles are doing what. Injecting without watching the face move is how asymmetry happens.
  2. Brow position check: measured before any forehead treatment. A patient relying on their frontalis to hold a descended brow should not have it relaxed.
  3. Reconstitution: the product is mixed with sterile saline in front of you. Dilution affects how far it spreads, and an over-diluted product spreads into muscles you did not intend to treat.
  4. Marking: injection points are marked on the skin, typically five points in the glabella, four to six across the forehead and three per side for crow's feet.
  5. Numbing: a cooling device or topical anaesthetic if you want it. Most patients do not — the needle is very fine.
  6. Injection: small volumes placed into the muscle belly at each marked point. Depth matters: too superficial and it spreads across the skin, too deep in the wrong place and it reaches muscles that should be left alone.
  7. Avoiding danger zones: injections are kept well away from the levator muscle of the upper eyelid, which if affected causes a drooping lid lasting weeks.
  8. Documentation: the brand, total units per area and batch number are recorded, so a future treatment can be adjusted from a known starting point.

After botulinum toxin: what to expect

There is no downtime. Small raised bumps settle within an hour, the effect begins at three to five days and is complete at two weeks.

  • Hour 0–1: small raised bumps at each injection point, like nettle rash. They settle within an hour.
  • Hours 0–4: stay upright. Do not lie down, and do not rub or massage the area — both can spread the product into unintended muscles.
  • Hours 0–24: no strenuous exercise, no alcohol, no hot baths or saunas.
  • Days 1–3: no visible change yet. This is normal and not a sign it has failed.
  • Days 3–5: the first softening becomes noticeable as the muscle begins to relax.
  • Day 14: full effect. Judge the result now, and raise any asymmetry while a small top-up can correct it.
  • Weeks 8–12: the effect begins to fade gradually as nerve endings regenerate.
  • Months 3–4: movement returns fully, and repeat treatment is due.

Is botulinum toxin safe?

At cosmetic doses it is one of the best-studied treatments in medicine with an excellent safety record. The risks are mostly cosmetic and all are temporary.

Risks you should know about

  • Eyelid droop (ptosis): the product reaching the levator muscle. Uncommon, lasts three to six weeks, and is usually caused by rubbing or lying down too soon.
  • Brow droop: from over-treating the forehead, particularly in patients with an already low brow.
  • Asymmetry: common and usually correctable with a small top-up at two weeks.
  • Frozen expression: from too many units, not from the product itself.
  • Bruising: at injection points, minor and short-lived.
  • Headache: for a day or two after treatment, in a minority.
  • Spock brow: the outer brow peaking oddly, from uneven forehead treatment. Easily corrected.
  • Antibody resistance: very rare at cosmetic doses, more likely with frequent high-dose treatment.

Tell your doctor about any neuromuscular condition, and about aminoglycoside antibiotics or muscle relaxants, both of which interact with botulinum toxin. Pregnancy and breastfeeding are contraindications.

Can botulinum toxin be combined with other treatments?

Constantly. Toxin treats movement, filler treats volume and surgery treats position — most faces benefit from more than one, and they address genuinely different problems.

The most common mistake in non-surgical aesthetics is using one treatment for every problem. A deep nasolabial fold is not a muscle problem and toxin will not touch it; a frown line is not a volume problem and filler is the wrong answer.

Commonly combined with

  • Dermal fillers: toxin for the upper face where movement creates lines, filler for the mid and lower face where volume has gone. See dermal fillers.
  • Skin boosters: for skin quality and fine crepey texture that neither toxin nor filler addresses. See skin boosters.
  • Facelift or brow lift: toxin afterwards maintains the surgical result by keeping the depressor muscles relaxed. Usually started three to four weeks after surgery.
  • Laser or peels: for pigmentation and texture, which are skin rather than muscle problems.
  • Lip lift: toxin to the depressor muscles at the mouth corners complements the change in lip proportion.

If you are having surgery with us, ask about toxin at your consultation rather than booking it separately. There is usually a sensible sequence, and doing it in the wrong order wastes money.

How to prepare for botulinum toxin

Avoid blood thinners for a few days, send photographs of your face moving, and plan not to lie down for four hours afterwards.

  • Photographs: send them with your face relaxed and then frowning, raising your brows and smiling. The movement photographs are what matter.
  • Blood thinners: avoid aspirin, anti-inflammatories, fish oil and vitamin E for a few days beforehand to reduce bruising.
  • Alcohol: avoid for 24 hours before and after.
  • Previous treatment: tell your doctor when you last had toxin, which brand and how many units. It changes the dose.
  • Medication: declare any neuromuscular condition, aminoglycoside antibiotics or muscle relaxants.
  • Timing: do not have it in the four hours before a flight if you can avoid it — you should stay upright, and seats recline.
  • Events: if you have a wedding or similar, have it at least three weeks beforehand so the full effect has settled and any top-up can be done.

Botulinum toxin results and how long they last

The full effect appears at two weeks and lasts three to four months. With regular treatment the interval often lengthens, because the muscles weaken with disuse.

A good result preserves expression. You should still be able to look surprised, concerned and amused — the lines simply do not etch as deeply when you do. The frozen look that makes botulinum toxin recognisable comes from dose rather than from the product, and a conservative first treatment that can be topped up at two weeks is far better than an aggressive one you have to live with for four months.

Longevity varies with dose, muscle strength and metabolism. Strong muscles in men and in heavy exercisers often metabolise it faster. Patients who treat regularly over years frequently find the interval stretching from three months to four or five, because muscles that have not contracted strongly for years genuinely become weaker. Starting treatment before lines become etched is the single most effective preventive step available in facial ageing.
