What is an unshaven hair transplant?

An unshaven transplant extracts grafts without shaving the whole donor area and places them between your existing hairs, so there is no obvious sign that anything has been done.

There are three levels of "unshaven", and clinics are not always clear about which they are selling. Understanding the difference is the single most useful thing on this page.

  • Fully unshaven (U-FUE): nothing is shaved at all. Grafts are extracted from between long donor hairs one by one and placed between existing recipient hairs. The most discreet and the most limited — typically up to 1,500–2,000 grafts.
  • Partially unshaven: a narrow strip or window of donor hair is shaved and the longer hair above is combed down to cover it. The recipient area is left unshaven. Allows more grafts, up to around 2,500, and is invisible once your hair falls naturally.
  • Recipient-unshaven only: the donor area is fully shaved but the recipient area is not. Useful if you have enough length on top to hide the work, but the back of your head is visibly shaved.

Most packages sold as "unshaven" are the partial version, and that is usually the sensible choice — it gives genuine discretion without reducing graft numbers as severely as a fully unshaven session.

Why it costs more and does less

  • Extraction is slower: working between long hairs means the surgeon cannot see the follicle exit angles as clearly, so each graft takes longer and demands more care.
  • Placement is slower: grafts must be threaded between existing hairs without damaging them.
  • Graft numbers are lower: a full day of unshaven work produces fewer grafts than the same day shaved.
  • Transection risk is higher: an unshaven field makes it easier to cut a follicle by mistake, which is why operator experience matters more here than anywhere else.

    You send photographs of your hairline, crown and donor area, plus a photograph showing your current hair length. A surgeon confirms whether your graft requirement fits within an unshaven session and you receive a written fixed quote stating exactly what, if anything, will be shaved.

  2. Day 1Arrival

    Your driver meets you at Istanbul airport and takes you to the hotel. Come with your hair at its normal length.

  3. Day 2The procedure

    Blood tests, then the hairline is drawn with you in front of a mirror. If a donor window is being shaved, it is marked and shown to you first. Extraction takes four to five hours — slower than shaved work — then placement between your existing hairs.

How we verify a facility

How is an unshaven hair transplant performed?

Seven to nine hours under local anaesthetic. The stages are the same as FUE, but every one of them takes longer because the surgeon is working between long hairs.

  1. Hairline design: drawn with you sitting upright in front of a mirror, accounting for your existing hair and how it falls.
  2. Donor preparation: for a partially unshaven procedure, a narrow window or several small strips are trimmed within the donor area and the longer hair above combed down to cover them. For a fully unshaven procedure, nothing is cut.
  3. Local anaesthetic: injected into the donor area. The only genuinely uncomfortable part, lasting a couple of minutes.
  4. Extraction: follicular units are removed one at a time with a fine punch, worked between the surrounding long hairs. This is the slow stage — four to five hours against three for shaved FUE — and demands significant operator experience to avoid transecting follicles.
  5. Trimming the grafts: each extracted graft carries a full-length hair, which is trimmed to a manageable length before placement.
  6. Sorting and storage: grafts are sorted by hair count and held in chilled preservation solution.
  7. Placement: grafts are threaded between your existing hairs, either into channels opened with a fine blade or directly with a Choi implanter pen. Care is taken not to damage surviving follicles.
  8. Dressing: a light dressing on the donor window and a headband to control forehead swelling.

Unshaven transplant recovery timeline

The biological recovery is identical to shaved FUE. The difference is cosmetic: your existing hair hides much of it, so most patients are presentable within days rather than weeks.

  • Days 1–3: tightness in the donor area and forehead swelling, which can reach the eyes around day three. Harmless and settles within 48 hours.
  • Days 2–4: most unshaven patients are comfortable back at a desk. Redness between the hairs is the main visible sign, and existing hair covers much of it.
  • Days 3–10: you wash daily with the lotion and shampoo provided. Washing is fiddlier with long hair — the clinic teaches you the technique before you fly.
  • Days 10–14: crusts clear. The donor window, if one was trimmed, is already growing back and covered.
  • Weeks 2–4: shock loss — the transplanted hairs shed. Completely normal; the follicle is intact. Existing hair around them may also shed temporarily.
  • Months 3–4: first new growth. Months 6–12: thickening continues.
  • Months 12–18: the final result.

Is an unshaven hair transplant safe?

The risk profile is the same as FUE, with two technique-specific concerns: higher follicle transection during extraction, and more shock loss of existing hair during placement.

Risks you should know about

Can an unshaven transplant be combined with other treatments?

Yes — and PRP and medical treatment matter more here than usual, because you are placing grafts into an area that still has hair worth protecting.

When grafts go between existing hairs rather than into a bald area, protecting those existing hairs becomes part of the plan. Shock loss is more likely, and medical treatment reduces both the extent and the duration of it.

Commonly combined with

  • Finasteride and minoxidil: the two treatments with the strongest evidence for slowing pattern loss, and the best protection against shock loss becoming permanent.
  • DHI placement: the Choi pen threads between existing hairs more easily than a blade-and-forceps approach, which is why unshaven DHI is a common pairing.
  • A second session later: for patients who need more than 2,500 grafts but will not shave, two unshaven sessions twelve months apart is a workable plan.

If your requirement is well above 2,500 grafts, be honest with yourself about whether two unshaven trips at £3,000 each is better value than one shaved session at £2,400.

How to prepare for an unshaven transplant

Grow your hair to a usable length, confirm in writing what will be shaved, and stop blood thinners and alcohol before you travel.

  • Hair length: your coordinator will tell you the ideal length — usually enough on top to comb over the recipient area, and enough at the back to cover a trimmed window. Too short defeats the point; too long makes extraction harder.
  • Get it in writing: confirm exactly what will and will not be shaved before you pay a deposit.
  • Medication: tell your surgeon everything you take. Blood thinners, aspirin and some supplements raise bleeding during extraction.
  • Alcohol and smoking: no alcohol for 3 days before, and stop nicotine for at least 2 weeks before and after.
  • Minoxidil: stop about a week before and restart when your surgeon says so.
  • On the day: wash your hair, use no gel or wax, and wear a shirt that buttons at the front.

Unshaven transplant results

The result is the same as any FUE: permanent, natural hair that reaches its final density between twelve and eighteen months. What differs is how little anyone knew it was happening.

Judge the result at twelve months, not before. The one difference worth noting is that unshaven results can look briefly worse than the starting point at weeks three to six, because shock loss affects both the transplanted hairs and some of the existing ones around them. This phase is temporary and genuinely unsettling if nobody warns you about it.

Because unshaven sessions move fewer grafts, expect improvement rather than transformation from a single session. For a Norwood 2 or 3 that is often exactly enough.
