Direct hair implantation loads each follicular unit into a Choi implanter pen — a hollow needle with a plunger — which opens the recipient channel and releases the graft into it in one movement, rather than opening channels first and placing grafts afterwards.
Extraction is identical to FUE: units are removed one at a time with a fine punch from the donor area. The difference is entirely in how they go back in. In FUE the surgeon opens all the channels first, then a team places grafts into them. In DHI there is no separate channel-opening stage — the pen does both at once.
That changes two things in practice. The graft spends less time outside the body, which theoretically improves survival. And because the pen controls depth and angle mechanically, the surgeon has very fine control over each individual hair — which matters most at the hairline, where a single misangled graft is visible.

Two reasons, both legitimate. The session takes two to three hours longer because each graft is loaded individually into a pen. And the Choi tips are single-use consumables — a full session uses dozens of them, and they are not cheap. If a clinic offers DHI at the same price as FUE, ask what they are actually doing.

You send photographs of your hairline, crown and donor area. A surgeon assesses your Norwood stage, donor density and graft count, and recommends DHI or FUE with reasons. You receive a written fixed quote.
Your driver meets you at Istanbul airport and takes you to the hotel.
Blood tests first, then your surgeon draws the hairline with you in front of a mirror. The donor area is shaved; the recipient area often is not. Extraction takes about three hours, then lunch, then placement with the Choi pens — the longest stage. Expect to be in the clinic eight to ten hours.

Judge the result at twelve months. At three months you will see almost nothing; at six you will see enough to be encouraged; at twelve the density is close to final, and the crown may still improve into the eighteenth month. DHI's advantage usually shows at the front — a hairline with a softer, more irregular edge that reads as natural at close range.
