A BBL is two operations in one: liposuction to harvest fat from the waist, abdomen, flanks or thighs, and fat grafting to inject that purified fat into the buttocks. The waist narrows and the buttocks gain volume, and much of the visual effect comes from that contrast.
It is not a lift in the surgical sense — no skin is removed and nothing is raised. The name is marketing rather than anatomy. What actually happens is that fat is moved from where you do not want it to where you do, which is why a BBL on a very lean patient is difficult: there has to be enough donor fat to harvest in the first place.
BBL has historically carried the highest mortality rate of any cosmetic procedure — at one point estimated at around 1 in 3,000, far above any other elective aesthetic operation. The cause is specific and well understood: fat injected into or beneath the gluteal muscle can enter the large veins that run through it, travel to the lungs and cause a fatal pulmonary fat embolism.
That risk is now substantially reduced, and the reason is a change in technique rather than better rescue. Injecting only into the subcutaneous layer above the muscle, using a blunt cannula of at least 4 mm, keeping the cannula angled upward and never aiming downward into the muscle, and in many practices using real-time ultrasound to confirm cannula position — these measures have brought reported mortality down dramatically. Valentmedica refers BBL only to surgeons who work this way.

Longer than most of our body procedures, for two reasons. BBL combines extensive liposuction with fat grafting, so the fluid shifts and clot risk are higher than for either alone. And the first week is when a fat embolism or a significant infection would declare itself. Packages that fly you home on day four are not accounting for either.

Surgeons deliberately over-inject to account for this, which is why the immediate result looks bigger than the agreed goal. Judging the outcome before six months is misleading in both directions — too large early, and still settling at three. The twelve-month photograph is the honest one.
What survives is permanent, because transplanted fat cells behave like the fat they came from and establish their own blood supply. That also means they respond to your weight: gaining weight enlarges the buttock, losing it shrinks it. The patients who keep the best results are the ones who reached a stable weight before surgery and held it afterwards, protected the graft by not sitting for three weeks, and did not smoke.
