A mastopexy removes excess skin, tightens the breast tissue beneath and moves the nipple to a higher, more natural position on the breast mound. The volume stays the same — what changes is where it sits.
Breast ptosis is measured by one thing: where the nipple sits relative to the inframammary fold, the crease beneath the breast. If the nipple is above the fold, you probably do not need a lift. Level with it, you are borderline. Below it, no implant alone will give you the result you want, because adding volume to a dropped breast makes it heavier and lower, not higher.
This single measurement is the most useful thing to understand before any consultation, because it determines whether the honest recommendation is a lift, an implant, or both — and clinics that recommend an implant alone for a clearly ptotic breast are selling the simpler operation rather than the right one.

More lift means more scar. There is no technique that produces a significant lift without a vertical scar, and any clinic promising one is either planning a periareolar lift that will not achieve what you want, or not being straight with you.

Where your nipple sits relative to the crease beneath your breast. Above it, an implant alone may be enough. Level with it, you are borderline. Below it, an implant alone will make the breast heavier and lower — you need a lift.
It depends on how much lift you need. A small lift scars around the areola only. A moderate one adds a vertical line down to the fold. A large one adds a horizontal scar in the fold as well. More lift means more scar, and no technique avoids that.
